We are currently accepting patients in our Clinic on a walk-in, no referral required, basis.

Prior to your visit, you are encouraged to complete and submit a Medical Questionnaire  on my.shouldice.com.  If you are unable to submit this document in advance, you will be asked to complete it upon arrival at the clinic.  Please bring your health card, your family doctor contact information, and details of prior surgeries and current medications.

To ensure the safety of our employees and patients, do not come to the Clinic if you are feeling unwell, have any COVID-19 symptoms, or have come into contact with anyone suspected of having COVID-19.

All patients will be actively scanned at the front door.  Should Clinic volumes necessitate due to physical distancing restrictions, you may be asked to wait in your car - please bring a cell phone to allow us to contact you.
